To be effective under § 512(c)(3), your notice must include:

• Your physical or electronic signature.
• Identification of the copyrighted work you claim has been infringed.
• Identification of the material that is claimed to be infringing, and enough information for us to locate it (e.g. the URL).
• Your name, mailing address, telephone number, and email address.
• A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
• A statement, made under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or are authorized to act on the owner’s behalf.

If your material was removed or disabled and you believe that was a mistake or misidentification, you may send a counter-notice to the same agent containing the elements required by § 512(g)(3). We will forward a valid counter-notice to the original complainant; absent a court action, we may restore the material in 10 to 14 business days.

We maintain a policy of terminating, in appropriate circumstances, the accounts of users who are repeat infringers.
